Thank you for trusting Desert Pain Institute with your care. Our main focus is to relieve your pain and help you to live a fuller life. Our team of doctors, nurses, physical and occupational therapists and our entire administrative staff are committed to offering you the best care and service available using state of the art treatments and facilities. We strive to devote all our resources to offering you the best care and service possible.

Important Information Required by Your Doctor
To help us provide you with the best care and service you are required to:
- Arrive at appointed check-in time
- Bring all forms completed
- Bring insurance card(s)
- Bring driver’s license or other picture I.D.
- Bring any previous medical records relating to your pain and/or current condition
- Bring films and reports from any MRIs, CT scans or X-rays relating to your pain and/or current condition
- Bring all prescription medication and/or herbal supplements you are currently taking (please bring the bottles)
- Bring payment (if applicable); we accept cash, check, MC, and/or Visa
- Please do not expect a refill of pain medication on your first visit
