To help us provide you with the best care and service are required to:

• Arrive at appointed check-in time
• Bring all forms completed
• Bring insurance card(s)
• Bring driver’s license or other picture I.D.
• Bring any previous medical records relating to your pain and/or current condition
• Bring films and reports from any MRIs, CT scans or X-rays relating to your pain and/or current condition
• Bring all prescription medication and/or herbal supplements you are currently taking (please bring the bottles)
• Bring payment (if applicable); we accept cash, check, MC, and/or Visa
